    The questionnaire takes approximately 20 minutes to complete and is 100% confidential. Your employer will never see the results.
  1. View your personalized health report and action plan.
    Once you complete the health assessment, you will receive an easy-to-understand report of your health strengths and risks. You will also receive next steps you can take to maintain or improve your path to good health.
  1. Receive your employer contribution.
    You will receive a 2018 wellness incentive in the form of a deposit to your HSA for employees enrolled in the HDHP’s or a health and wellness gift card for employees enrolled in the OAP the month after you complete the assessment. The employer wellness incentive amount is $200 for employee only medical coverage or $400 for all other medical coverage categories (i.e., employee + spouse, employee + children, family coverage). The employer contribution is prorated for mid-year enrollees.
