Here’s How It Works

If you are enrolled in one of the High Deductible Health Plans with a Health Savings Accounts (HSA) and you complete an annual Biometric Screening, the Company will deposit an employer contribution to your Health Savings Account (HSA) in the amount of $250 for employee only medical coverage or $500 for all other medical coverage categories (i.e., employee + spouse, employee + children, family coverage). The Biometric Screening is fast, free, convenient and 100% confidential!

If you are enrolled in the OAP (Open Access Plan) and you complete an annual Biometric Screening, you will be eligible to receive a health and wellness gift card in the amount of $200 for employee only medical coverage or $400 for all other medical coverage categories (i.e., employee + spouse, employee + children, family coverage). Wellness gift cards can be used  at a variety of health and wellness merchant vendors.

Complete the Biometric Screening by November 30, 2020 to earn your wellness incentive. It only takes about 15 minutes to be screened, is completely FREE, and the results will not be shared with your employer! Why WOULDN’T you do it?

Scheduling a Screening

  • Biometric Screenings can be completed as a part of an annual physical with your doctor.
  • You can also access a wellness screening form by following the below:
    • Log into myCigna.com.
    • Click on the myHealth tab in the top left corner
    • click on “View my Incentive Details” in the middle of the page.
    • On the incentives page, click on “Goals”, and the incentives will be listed.
    • Under “Get a personalized biometric health screening”, click on “Download a wellness screening form” to print a copy of the form.
    • The form can be mailed or faxed to Cigna or uploaded within myCigna.com.  Instructions are listed on the form.
